An adverse incident (or adverse event) is an event or circumstance that occurs during the course of care that results in or has the potential to result in harm, significant safety concerns, or other situations requiring review, coordination, or follow-up.
Adverse incidents vary in severity and complexity. Some events require submission of an Adverse Incident Report, while others may warrant reporting when additional awareness, consultation, coordination, or follow-up would support client care, clinical decision-making, or continuity of care.
